Welcome to Emerge Weight & Hormone Specialists. By accessing our website at www.three.health, scheduling appointments, using our services, or becoming a patient, you agree to be bound by these Terms and Conditions. If you do not agree to these terms, please do not use our website or services.
These Terms and Conditions constitute a legally binding agreement between you and Emerge Weight & Hormone Specialists, located at 3500 188th ST SW Suite 250, Lynnwood, WA 98037 & 1313 E Maple St, Bellingham, WA 98225.

Emerge Weight & Hormone Specialists specializes in metabolic health, weight management, and hormone therapy. Our services include medical consultations, treatments, aesthetic services (EMFACE, EMSELLA, EMSCULPT NEO), wellness services, supplements, and other health-related products and services.

Medical Services and Provider-Patient Relationship
No Provider-Patient Relationship via Website
Using this website or viewing its content does not create a provider-patient relationship. A provider-patient relationship is established only when you schedule and attend an appointment with one of our healthcare providers and appropriate consent forms are signed.

Services and Eligibility
Types of Services
We offer:

- Telehealth services (available to patients located in Washington State)

Eligibility
Our services are available to adults age 18 and older. Minors (under age 18) may receive services only with parental or legal guardian consent and accompaniment.
Geographic Limitations
Telehealth services are only provided to patients physically located in Washington State at the time of the appointment. In-person services are available at our Lynnwood, WA & Bellingham, WA locations.
Appointments and Scheduling
Appointment Scheduling
Appointments may be scheduled by calling our office at (425) 606-0022 or through our online scheduling system (if available).
Cancellation Policy
We require 24 hours advance notice for appointment cancellations or rescheduling. A $75 fee will be charged for:
- Appointments cancelled with less than 24 hours notice
- Missed appointments (no-shows)
This fee applies to all appointment types, including in-person and telehealth visits. The fee will be charged to the payment method on file and is not billable to insurance.
Emergency Exceptions: If you experience a true emergency that prevents adequate notice, please contact our office at (425) 606-0022 to discuss your situation.
Provider Discretion
Our providers reserve the right to refuse service or terminate the provider-patient relationship at any time for appropriate reasons, including but not limited to non-compliance with treatment plans, disrespectful behavior toward staff, failure to pay for services, or violation of office policies.
Payment and Financial Policies
Accepted Payment Methods
We accept cash, personal checks, major credit cards (Visa, MasterCard, American Express, Discover), debit cards, HSA cards, and FSA cards.
Insurance-Based Services
For services covered by insurance (beginning January 2026), we will verify your insurance benefits and bill your insurance company directly. However:
- You are ultimately responsible for all charges, regardless of insurance coverage
- Co-payments, deductibles, and coinsurance are due at the time of service
- If your insurance denies a claim, you are responsible for the full balance
- You are responsible for understanding your insurance benefits, coverage limitations, and exclusions
Insurance Verification: Verification of benefits is not a guarantee of payment. Insurance companies may deny claims for various reasons after services are rendered.
Cash-Pay Services
Certain services are offered on a cash-pay basis only and are not billed to insurance, including:
- Aesthetic treatments (EMFACE, EMSELLA, EMSCULPT NEO)
- Certain wellness services
- Specific supplements and products
- Other services designated as cash-pay
Payment for cash-pay services is due in full at the time of service.
Outstanding Balances
All balances are due within 30 days of the statement date unless other payment arrangements have been made in advance with our billing department. Patients with outstanding balances past 30 days may be required to pay the outstanding balance in full before scheduling future appointments.
Collections
Accounts over 90 days past due may be forwarded to collections. You will be responsible for all collection costs, including collection agency fees, attorney fees, and court costs. Collection activities may be reported to credit bureaus as permitted by law.
Payment Plans
If you are unable to pay your balance in full, you may request a payment plan before the account is 90 days past due. Contact our billing department at (425) 606-0022 to discuss payment plan options.
Membership Programs
Membership Terms
All membership fees are due in full at the time of enrollment. Memberships may be set up on automatic payment through credit card or ACH bank draft.
Failed Payments
If an automatic payment fails, you will be notified immediately and given 5 business days to provide alternative payment. A $25 failed payment fee may be assessed.
Membership Termination
If your membership payment remains unpaid for more than 30 days past the due date, your membership will be automatically terminated and any accumulated credits, benefits, or perks will be forfeited.
Membership Cancellation
If you request to cancel your membership, all membership benefits, services, and prescription refill privileges will terminate immediately upon your cancellation request, regardless of any remaining days in your current membership period. Membership fees are non-refundable.
Refund and Return Policy
Services Already Rendered
No refunds will be provided for services already performed, including medical consultations, treatments, procedures, aesthetic services, laboratory tests, telehealth visits, and other professional services rendered.
Services Not Yet Rendered
- Within 90 days of purchase: Refunds may be issued for services that have not yet been performed
- After 90 days: Prepaid services will remain as credit on your account with no expiration date
Products
Products may be returned or exchanged within 14 days of purchase with proof of purchase. Products must be unopened and in original packaging. Opened or used products cannot be returned for health and safety reasons. Prescription items are non-returnable.
Defective Products
If you receive a defective product, notify us within 7 days of purchase for replacement or full refund.
Refund Processing
Approved refunds will be processed within 7-10 business days to the original payment method.
Telehealth Services
Telehealth Availability
Emerge offers telehealth services in compliance with Washington State laws. Telehealth appointments are available only to patients physically located in Washington State at the time of the appointment.
Technology Requirements
To participate in telehealth appointments, you need a device with camera and microphone, reliable internet connection, and a private location.
Privacy and Security
We use secure, HIPAA-compliant video conferencing platforms. However, no technology is completely secure. You are responsible for ensuring you are in a private location during telehealth appointments.
Limitations
Telehealth has limitations including limited physical examination, inability to perform certain diagnostic tests remotely, and potential technical issues. Your provider may determine that an in-person visit is necessary.
Privacy and Data Protection
HIPAA Compliance
We comply with the Health Insurance Portability and Accountability Act (HIPAA) and maintain the privacy and security of your protected health information. For detailed information about how we use and protect your health information, please see our Notice of Privacy Practices and Privacy Policy.
Electronic Communications
We may communicate with you via email and text/SMS for appointment reminders, test results, billing statements, and other healthcare-related communications. By providing your contact information, you consent to receive these communications.
Security Risks: Email and text messaging are not completely secure. We cannot guarantee confidentiality of information transmitted via email or text. You may opt out at any time by contacting our office.
Marketing Communications
We may send marketing communications including newsletters, product promotions, and service updates. You may opt out at any time. Your decision to receive or decline marketing communications will not affect your access to care.
